About This Tool
This checker covers the most clinically significant psilocybin drug interactions based on case reports, pharmacological mechanisms, and harm reduction consensus. It is not exhaustive — if your medication is not listed, that does not mean it is safe to combine.
Always disclose all medications and supplements to your licensed facilitator before a session. Oregon and Colorado facilitators are required to conduct medication screening. Clinical trial participants undergo extensive medication review before enrollment.
Hard contraindications
The contraindicated substances — lithium, MAOIs, and tramadol — represent genuine do-not-combine situations with documented serious adverse events. These are not risk-benefit situations; they are situations where combining the substances carries unacceptable risk of severe harm.
SSRIs and antidepressants
The SSRI interaction is the most commonly encountered and most complicated. SSRIs blunt psilocybin effects rather than creating danger. The risk is in how people respond: stopping SSRIs abruptly to pursue psilocybin can trigger discontinuation syndrome, which is dangerous. Discuss any medication changes with your prescribing physician.
